Adult GH Deficiency (AGHD) is characterized by an altered body composition, an atherogenic lipid profile, decreased exercise capacity, and diminished quality of life. We performed a randomized, double-blind, placebo-controlled, multicenter study in 166 subjects with AGHD to assess the effects of GH on these outcomes. GH was initiated at 0.0125 mg/kg·d, increased to 0.025 mg/kg·d as tolerated, or decreased to 0.00625 mg/kg·d for 12 months. Primary measures of efficacy included body composition, strength and endurance, and quality of life. Additional parameters included serum IGF-I concentrations, serum lipids, and bone mineral density.

After 12 months, 79% of subjects remained on GH 0.0125 mg/kg·d, whereas 21% received 0.00625 mg/kg·d. GH-treated men and women demonstrated significant decreases in total body and trunk fat and increases in lean body mass over baseline. In GH-treated men, mean IGF-I sd scores exceeded age-adjusted normal ranges, whereas similar doses produced a smaller response in women. GH treatment was associated with significant improvements in total cholesterol and low-density lipoprotein (P < 0.05 for all). No significant treatment effects were observed in strength and endurance, quality of life, or bone mineral density. GH treatment was generally well tolerated. Subjects with AGHD should receive individualized GH therapy to maintain IGF-I between the mean value and +2 sd and improve body composition and cardiovascular risk factors.

However, GH secretion occurs throughout life and is known to have profound effects on anabolism, lipolysis, and carbohydrate metabolism. Individuals who develop adult GH deficiency (AGHD) secondary to pituitary or hypothalamic disease exhibit abnormal body composition, characterized by a significant increase in fat mass, particularly visceral fat, and a decrease in lean body mass (1, 2, 3, 4, 5, 6). Many have diminished strength and exercise capacity that may improve with GH therapy (7, 8). Adults with AGHD also demonstrate altered lipid metabolism, increased incidence of cardiovascular disease, and diminished quality of life (9, 10, 11, 12). Reduced life expectancy secondary to increased cerebrovascular and cardiovascular disease has also been reported in patients with hypopituitarism (13). Adults with AGHD experience feelings of social isolation, decreased energy, and an overall poorer quality of life when compared with controls (11, 14, 15, 16). Bone mineral density (BMD) is also reduced in these subjects (6), resulting in a 3-fold increase in bone fracture rate (17).

Short-term GH replacement therapy in adults has been associated with beneficial effects on body composition, fat distribution, and quality of life (2, 18, 19). Improvements in bone mineral content and BMD are not apparent until GH has been administered for at least 18 months (20, 21). In general, studies of GH therapy in subjects with AGHD have been relatively small, nonrandomized, or uncontrolled. Moreover, potential differences in the ability of men and women to respond to GH have not been adequately examined in most of these studies.

This large multicenter, randomized, placebo-controlled study explored the effects of GH treatment on body composition and physical performance. The primary end points were a decrease in percentage of body fat, an increase in muscle strength, and improved quality of life. The anticipated reduction in cardiovascular risk emerged as the most compelling reason to examine the impact of GH replacement therapy in adults with GHD.

Dr Lionel Bissoon is one of America’s leading anti-aging gurus. The rich and famous queue up at his New York clinic to be treated with the drug.

‘For many, this is the fountain of youth and it’s very, very popular,’ he says. ‘My patients say they look good, feel strong and even sleep better. Very few people are disappointed.’

Of course, the ministrations of Dr Bissoon do not come cheap: regular treatment can cost up to £1,200 a month. And therein lies the rub. The vast majority of those using HGH both here and in America cannot afford such costs. Instead, they purchase it over the internet from foreign-based websites and then inject themselves.

In so doing, they blindly trust that what they are sent actually does contain some HGH — rather than being counterfeit or contaminated with unknown substances that could harm or even kill them.

But that isn’t the only risk. HGH is not licensed as an anti-aging drug. While some studies have suggested it can help the body heal itself and improve athletic performance, other experts believe its anti-aging abilities have been wildly exaggerated.Worse still, there are growing concerns about the dangers that HGH may pose to health.

Studies show that high levels can lead to swelling of the soft tissues in the body; abnormal growth of the hands, feet and face; high blood pressure; blood clots; diabetes; increased sweating; and excessive hair growth.

Organs — including the heart, liver and kidneys — may also undergo excessive growth, leading to potentially life-threatening problems such as cardiomyopathy, a disease of the heart muscle. Cancer risks are also thought to increase.

Given that, many will be surprised at just how easy HGH is to obtain in this country.

Manufactured under trade names such as Saizen, Genotropin, Norditropin, Humatrope, Zomacton and Nutropin aq, there are dozens of websites offering it for sale.

The vast majority of these are unregulated and based abroad, thereby sidestepping any risk of prosecution under British laws.

Like steroids, HGH is a class C drug which is illegal to supply. It should only be available from a doctor on prescription to treat severe growth disorders or conditions such as Aids-related muscle wastage. But at the same time, it is not illegal to possess, if possession is for personal use.

In other words, if someone is caught with relatively small amounts of HGH, they are unlikely to have any action taken against them.

And so the internet has become the main channel into the UK and its use — once confined to athletes hoping to gain an illegal edge — has become widespread.

Only last year, a gym in Bristol had to install needle-bins in the men’s toilets because so many people were using the drug (one gym member told how empty vials of HGH had been found on the premises and how he had seen people openly injecting themselves. Syringes had been left in the changing rooms and even on the gym floor.)

Meanwhile, online keep-fit forums are awash with people trading information and advice about the drug.

Last week, a man calling himself Big Chris posted the following: ‘I’m 21 about 18-19st — that’s fat not muscle. I used to be a bit of a gym buff, 13st of muscle but went to seed when I met my girlfriend. I’ve just started getting back into the gym and I’m considering going on HGH.’

Another wrote: ‘I’m considering getting some HGH — reason being is a few of my fellow gym buddies are on it and say it’s great, they are losing weight and have more energy, fewer aches and pains. What would you expect to pay for HGH? I’m about to pay  for ten days’ supply.’

What this man is about to purchase will no doubt have been imported from abroad. And, more worryingly, he will have no idea as to what it is he is about to start injecting into himself.

The dangers of this Russian roulette approach to self-medication were highlighted last year in a series of raids by police and officers from the Medicines And Healthcare Products Regulatory Agency (MHRA).

Targeting websites selling drugs like HGH, they seized more than £500,000 worth of medicines.

Tests on the confiscated pharmaceuticals showed high levels of counterfeiting and contamination with other drugs, suggesting those making them fail to clean equipment in between manufacturing runs.

‘The dangers of purchasing medicines from an unregulated source are that you just don’t know what you are taking,’ said Mick Deats, MHRA head of enforcement.

‘The dosages could be either too high or too low, contain no pharmaceutical ingredient or a totally different ingredient to that stated. Illegal suppliers have no quality control or standards to abide by.

‘If customers could see the filthy  conditions in which some of these medicines were being transported, stored and handled, they wouldn’t touch them.’

But it is not just gym-users who are taking HGH. The anti-aging industry in the US is worth 88 billion and in the UK worth £300 million a year. As in America, private clinics are offering the drug as a treatment.

But there is still a surprising  reluctance — in this country, at least — to talk about the treatment.

HGH is big news in anti-aging,’ said a Surrey-based doctor whose website advertises the anti-ageing benefits of HGH, but who asked not to be named.

‘But I won’t use it for anyone who isn’t actually deficient in it. To establish that means expensive blood tests, which will cost the patient over £500.

‘After that, if you have a dose of 0.4mg a day, it will cost about £500 a month, possibly up to £1,000 depending on the practitioner. After that, you will need frequent monitoring, which means more blood tests — and more money. It can run into thousands a year.

‘Most people who call me are taken aback by the price. They tell me they can get it much cheaper on the internet but, as I tell them, if they buy from anyone who isn’t a genuine medical doctor, which I am, they don’t know what they are getting into.’

‘It certainly didn’t make me look any younger. But even the positive effects were temporary. I lost some fat, but not my wrinkles. My feeling is that the benefits have been exaggerated’

One person who has first-hand experience of its effects is Dr Cecilia Tregear, a Harley Street medic who specializes in hormone treatments.

‘I took HGH several years ago,’ she admits. ‘I tried it when I turned 50 to find out if all the fuss was justified. What I noticed were initial, apparently positive effects on my energy levels and my libido rather than physical changes.

‘It certainly didn’t make me look any younger. But even the positive effects were temporary. I lost some fat, but not my wrinkles. My feeling is that the benefits have been exaggerated.’

Her view is backed up by a review of all the research into HGH conducted by scientists at Stanford University in California in 2007. It concluded that using the growth hormone resulted in small changes in body composition and an increased likelihood of conditions such as diabetes.

‘On the basis of this evidence,’ said the review, ‘growth hormone cannot be recommended as an anti-ageing therapy.’

But words such as that are unlikely to carry much truck with the likes of Graham Marks who believe that, in HGH, they have found the secret to eternal youth.

‘I know that I’m never going to be young again, but this stuff keeps me younger longer,’ he says. ‘I honestly feel it has taken at least a decade off my body age.’

Testosterone Replacement May Lower Risk of Heart Disease, Diabetes, Death, but Long-Term Effects Unclear

June 18, 2008 — Testosterone replacement therapy can help older men deficient in the hormone reduce their risks of heart disease, diabetes, and death, according to new research presented this week at ENDO 08, the annual meeting of The Endocrine Society.

”The goal of testosterone treatment is to keep the levels within normal range,” says Farid Saad, director of scientific affairs for Bayer Schering Pharma in Berlin. Saad presented two of the studies at the meeting, held in San Francisco. Bayer Schering Pharma makes several testosterone products.

Low levels of testosterone are common with age, Saad says, occurring in about 18% of 70-year-olds. Low levels of testosterone, he says, are associated with the metabolic syndrome — a cluster of risk factors such as abnormal cholesterol and high blood pressure that boost risk of heart disease, stroke, and type 2 diabetes as well as other risks to health.

In the studies, Saad and his colleagues found that testosterone replacement therapy reduced the metabolic syndrome risk factors and did so in a similar way in all the age ranges studied.

Testosterone Replacement Therapy and Heart Disease Risks

In the first study, Saad’s team looked at 95 men, aged 34 to 69, with low levels of testosterone.

All had metabolic syndrome. Those who have this diagnosis must have three of five risk factors: increased waist circumference, low “good” cholesterol or HDL, high triglycerides, elevated blood pressure, and elevated blood sugar.

“We treated them for at least a year,” he tells WebMD. Every three months, they measured cholesterol, waist circumference, and other parameters.

The testosterone replacement was given as a long-acting injection, every three months. The same product is not yet available in the U.S., Saad says, although other types are.

Testosterone Replacement Therapy: Test Results

“We saw a consistent improvement of the parameters of metabolic syndrome,” Saad says. The men were not given a special diet or exercise program.

The supplemental testosterone reduced total cholesterol, “bad” LDL cholesterol, triglycerides, and body mass index while improving “good” HDL cholesterol. The men lost their pot bellies, Saad says. “What we see after 12 months is a reduction in two or three trouser sizes, three or four inches off the waist. We see a reduction by one-fourth to one-third of their total cholesterol.”

No adverse effects were reported, he says.

While hormone replacement therapy for women has been found to be associated with increased risks of heart disease and other problems, Saad doesn’t foresee that will be the case with testosterone replacement. “There are fundamental differences between hormone replacement in women and testosterone treatment in men,” he tells WebMD.

Testosterone Replacement Therapy: No Age Effects?

In a second study, Saad divided the same 95 men into three groups, based on age: less than 57, 57 to 63, and older than 63.

They found the older men and the younger men had similar improvement in their risk factors.

“There are some precautions with testosterone supplementation,” Saad tells WebMD. “We need to monitor the prostate.”

“It is well known that with prostate cancer, the cancer is usually dependent on testosterone. Prostate cancer is a slow-growing tumor.” Cancer of the gland must be ruled out before starting supplements, he says. He also advises routine prostate checkups while on treatment.

A test to monitor red blood cell formation, called a hematocrit, is needed, too. “Testosterone increases red blood cells,” he says. In excess, it can theoretically boost heart attack or stroke risk.

Testosterone and Death Risk

Low testosterone levels are associated with an increased risk of death, according to Robin Haring, a researcher from the Institute for Community Medicine at Ernst-Moritz-Arndt University in Griefswald, Germany, who presented the finding at the meeting.

He evaluated data on nearly 2,000 men, aged 20 to 79, who participated in the Study of Health in Pomerania, following them for seven years until August 2007. He noted testosterone levels, age, weight, smoking habits, and physical activity. The drugmaker Novo Nordisk partly funded the study.

During the follow-up, 226 men died. “Men with low testosterone have a more than twofold higher risk of death during the follow-up period,” he says.

They were more likely to die of cardiovascular disease and cancer, but not of other causes.

Testosterone Replacement Therapy: Long-Term Effects Unknown

Another researcher, Jane F. Reckelhoff, PhD, professor of physiology and biophysics at the University of Mississippi Medical Center in Jackson, offers a caveat about supplemental testosterone: “We don’t know what the long-terms effects are. The safety studies have not been done and they need to be done.”

She reviewed the studies for WebMD and has published on a review about testosterone supplements for the American Journal of Physiology Renal Physiology.

Her chief concerns: “Testosterone [in excess] can increase blood pressure and compromise kidney function.”

But an advocate of testosterone replacement therapy, Martin Miner, MD, co-director of the Men’s Health Center at the Miriam Hospital of Brown University in Providence, R.I., says the studies show that restoring testosterone when it is low improves metabolic syndrome factors and could help prevent type 2 diabetes and cardiovascular risks.

He agrees close monitoring of men on testosterone is needed. “Checking hematocrit, PSA (prostate-specific antigen), and lower urinary tract symptoms in men is vital to the management of testosterone replacement therapy,” he says
